Physicals due for all freshman (and new transfers)
Make sure your FinalForms account is up-to-date!
- Upload your physical to FinalForms. You must have one on file before starting school or sports.
- Select sports your student is interested in. Don’t wait for the season to start, indicate interest as soon as possible! This makes sure you get early communications from the coaching staff about practices, gear, and meetings. You can check as many as you wish, and it is easy to update if he changes his mind. It is not a commitment, just a request for more information as it becomes available.
- Make sure you are all green. Green means you have completed an item in FinalForms. Participation with a team will not be permitted without all the forms being signed. Red = parent signature, yellow = student signature.
