Please ensure you update your FinalForms sports selection and complete all forms required for your sport. (These will unlock when you make your sport selection for the 2024-2025 year.)

Per Sac-Joaquin Section rules, athletes are not eligible to begin practicing with their high school team until after they have graduated from the 8th grade. This may affect some of our players, so if it applies to you, please wait until after your graduation to attend our workouts. There is no penalty for missing the voluntary workouts.

Are you cleared to start workouts?

• Log into FinalForms 
• Make sure all parent and student forms are signed
• To start practicing or conditioning, a valid physical must be on file clearing your son for participation, and all e-forms signed.  
  • Red = parent signature
  • Yellow = student signature
  • Green = All forms are signed and good to go
     
  • Please note that if a form requires both parent and student signature, the student CANNOT sign his forms until the parent signs/completes their set.
  • By signing these forms, your signature acknowledges that you have read and understand the content of the form
• (Class of 2028) In addition to a valid physical uploaded to FF and reviewed for clearance, our school registrar, Mrs. Case, must have a copy of your son’s immunization record.  She is managing this record clearance through FinalForms.  If you have not submitted your son’s immunization records, your son may not be on campus.
• (Class of 2028) Student has graduated 8th grade
