Athletic Forms

As we update our policies, we want to ensure that we have all of the proper paperwork on file in accordance with CIF rules and regulations. There are two new forms that will require student and parent signatures. When the new forms become available, you will be notified via email by Final Forms. At that time, please log into your account to sign and submit the new forms. Please note: when new forms become available in the Final Forms system, it will temporarily return your account status to ‘incomplete.’ As soon as you sign and submit the new form(s) through the system, your registration will once again return to 100% complete.

New forms added, signatures needed from parents and students

Student/Parent Handbook (2020-21)

  • Required for all students; both student and parent e-signatures required.
  • The 2020-21 handbook will show in red on your form-set indicating the need for review and signatures. The previous year’s handbook that you have already signed and acknowledged is still preserved in the form-set and is labeled “2019-2020 Student/Parent Handbook.” It will show in green, letting you know it is already complete and nothing further is needed.

Opioid Fact Sheet for Student-Athletes

  • Required for student-athletes only; both student and parent e-signatures required.

COVID-19 Waiver for Students

  • Required for student-athletes only; both student and parent e-signatures required.

To begin the process, parents must log in to their Final Forms accounts first to fill out and submit the necessary forms. After that, the system will alert students via email that they have new forms to sign. After both parent and student signatures are complete on all applicable forms, your son’s registration status on Final Forms will once again return to “complete.”

Parent Volunteer Drivers – DMV Form

If you wish to volunteer to help drive students to events, whether is is an athletic event, or other co-curricular event, you need to have a clean driving record with the DMV.

The following two items need to be submitted to your club  moderator (non-athletic) or Jesuit Athletics (Robin Cummings) if you are helping out an athletic team.

  • Filled out DMV form
  • A copy of your drivers license
